1.1.1.1 Indeling
Allo-antilichaam = gericht tegen antigenen ander individu
Herkomst
Auto-antilichaam = gericht tegen antigenen van jezelf
Natuurlijk voorkomende = aanwezig zonder contact met antigeen
Ontstaan
Immuun-antilichaam = aanwezig na contact met antigenen
Regulier antilichaam = Anti-A, Anti-B (IgM), van nature voorkomend
Irrigulier antilichaam = alle andere bloedgroepantigenen
Compleet antilichaam = IgM spontane agglutinatie
Serologie
Incompleet antilichaam = IgG agglutinatie via hulpmiddelen

Hemolytische transfusiereactie: patiënt heeft RBC-AL en krijgt transfusie
donorbloed toegediend, RBC donor: bloedgroep-AG waartegen AL zijn gericht
Vb patiënt met BG A (heeft anti-B) krijgt BG B ernstige reactie
Hemolytische ziekte van de neonatus: zwangere vrouw RBC-AL (IgG) tegen BG-
AG (vader) op RBC kind, IgG kan placenta passeren RBCkind afbreken roham
Auto-immuun hemolytische anemie: patiënt heeft BG-AL gemaakt tegen één of
meerdere BG-AG op eigen RBC RBC afgebroken RBC-tekort
Directe Coombs test is positief
Warme agglutinine (meestal IgG): ernstige problemen: kan RBC afbreken
elutie- of absorptiemethode: auto-AL weg en controleren op allo-AL
Koude agglutinine (meestal IgM): problemen bij bewaring bloed in koelkast
o Bij bloedtransfusie: moet bloed voorverwarmd worden (acceptor met koud)
o RBC wassen bij 37 °C geen aggregaten (donor met koude)

Natuurlijk voorkomende AL (anti-A en -B)
3 allelen: A, B en O
Antigenen: opgebouwd uit suikers
Elke bloedgroep heeft H-AG als fundering
Bloedgroep AG op RBC AL in bloed Bloed ontvangen van Bloed geven aan

B B-AG Anti-A B en O B en AB
AB A en B-AG Geen A, B, AB en O AB

Bloedgroepbepaling: (+) = agglutinatie reverse kan enkel met reguliere AL
