vooral pathologie van de mitralis- en aortaklep. Mitraalklep is het meest
belast (daar is het drukverschil het grootste)
Aortaklep en pulmonaalklep: passief. Zwaluwnestjes
Mitralisklep en TP: subvalvulair apparaat met chordae en papillairspieren
Oorzaken van kleplijden:
- Degeneratie: Aorta/mitralis (vooral oudere mensen)
- Ischemisch hartlijden: mitralis (door ischemie in de papillairspieren)
- Endocarditis: circulerende bacteriën hechten zich makkelijk aan de
klepblaadjes gaatjes daarin creëren. Kan op alle kleppen!
- Congenitaal: aorta/pulmonalis
- Dilatatie of dissectie aortawortel: als de aortawortel/vlak boven de
aortaklep dilateert, dan zal door de sinutubulaire junctie de
aortaklep zelf ook insuff worden! Dan moet je dus beide gaan
behandelen!
- Trauma: tricuspied: decelleratie trauma: klep die meest vooraan ligt
zal aangedaan zijn!
- Acuut gewrichtsreuma
Chirurgische mogelijkheden:
- Klepherstel: plastie
o Hier zullen we altijd het eerste voor gaan! Altijd mogelijk voor
de mitraalklep. Alles terug aan elkaar naaien en een vaste ring
errond naaien om te verhinderen dat die opnieuw zou
dilateren + draden om de chordae te herstellen
