CONGENITALE PATHOLOGIE- Evoluties:
o Prenatale diagnose:
▪ Informatie ouders

OBSTRUCTIEVE AANDOENINGEN = STENOSEN
- Leidt tot ventrikelhypertrofie
o Ischemie (coronairen kunnen de groei van het myocard niet volgen)
o Arrhytmieën
o Permanente spierschade
o Sudden death (ineens neervallen, plotse fibrillatie)
- Correctie! Je moet de stenose opheffen
- Diegene die we niet bespreken: pulmonaalstenose, HLHS, mitraalstenose, cor triatriatum
COARCTATIO AORTAE
- Zal leiden tot hypertrofie LV → gevaar
- Ontstaat door het weefsel van de ductus botalli dat in constrictie kan gaan na de geboorte:
fungeert als soort ligament. Klein beetje van endotheel za overgrijpen op de aorta → zal
coarctatio vormen
- Neonatale vorm:
o Kritisch, ontwikkeling tot shock (bij dichtgaan ductus) OL geen doorbloeding
o Eerst openen ductus botalli! Prostaglandines!
o Chirurgische correctie:
▪ Waldhousen
▪ End-to-end/extended end-to-end
▪ Reversed waldhausen
- Volwassen vorm
o Minder kritisch
o Ballondilatatie (stenting zo mogelijk)
o Chirurgie zo nodig
- CAVE: hypertensie indien gemeten in de BL → gevaarlijk op latere leeftijd
- R/
o Kind in slaap doen
o Op Re zijkant leggen en incisie maken links achteraan: 4de IC ruimte
o Long wegduwen en dan zie je parietale pleura: deze insnijden. Daaronder ligt de aorta
descendens
, o Thv aansluiting met de ductus botalli zal de coarctatio zitten.
o Let op voor zenuwen! (n. vagus)
o Heparine toedieen, want je zal vaatklemmen plaatsen. 1 boven en 1 onder de
coarctatio plaatsen
o 5 mogelijkheden
1. Patch innaaien: zone inknippen en een patch innaaien (zone zo breder). Doen we
niet meer! Nadien zullen kinderen hier aneurysma ontwikkelen.
2. Waldhousen subclaviaplastie: afbinden a. subclavia en doorknippen en inknippen
tot voorbij de stenose. Deze flap als patch gebruiken (zelfde zoals patch, maar dan
eigen weefsel). Kind zal goede collateralen ontwikkelen.
3. End-to-end: stuk eruit knippen en terug aan elkaar zetten.
