, Inleiding
In dit verslag wordt de module regisseren van zorg beschreven. De student liep op de afdeling
cardiologie in het ziekenhuis in Sneek stage tijdens het uitvoeren van de module.
Het doel van dit verslag is zorg te dragen van opname tot ontslag voor patiënten. Daarbij wordt nauw
samengewerkt met verschillende disciplines.
Een goede samenwerking tussen verschillende disciplines is belangrijk om tot een goed zorgproces te
komen. In deze module wordt beschreven wat de complexiteit voor deze module is. Het
multidisciplinaire zorgproces wordt uitgewerkt. Er wordt uitleg gegeven over gezamenlijke
besluitvorming en cultuur sensitieve zorg en hoe dit in de praktijk wordt ingezet. Daarna wordt er
een voorstel tot verbetering gegeven. De verschillende methoden van overdracht worden uitgewerkt
en toegelicht. Als laatste wordt de relevante wet- en regelgeving beschreven en hoe er aan
vakinhoudelijk leiderschap is gewerkt.
Bij elk hoofdstuk wordt eerst een stuk theorie uitgelegd en daarbij de uitwerking van dat hoofdstuk.
