,Inleiding
Dit verslag is een individueel begeleidingsplan, dat ik heb opgesteld met een cliënt buiten
mijn stage instelling om. Het was niet mogelijk om een cliënt van mijn praktijkleerplek uit te
kiezen, omdat er geen animo was voor individuele begeleiding. In dit begeleidingsplan heb ik
samen met een cliënt werkhypotheses opgesteld en hierbij aansluitende interventies bedacht
en uitgevoerd. Tijdens deze begeleidingsmomenten heb ik methodieken ingezet die afkomstig
zijn van de training “samenwerken in de specialistische hulpverlening”.
Het begeleidingsplan is uitgewerkt aan de hand van de methodische cyclus en bestaat
uit de volgende fasen: de oriëntatiefase, de definitiefase, de ontwerpfase, de uitvoeringsfase
en de evaluatiefase.
