VZ-B1-K1-W1O1 observatieplan.
De beginsituatie: De aanleiding van mijn observatie is: Mevr. K heeft een angststoornis die
de laatste maanden steeds verergerd. De Huisarts heeft antipsychotica voorgeschreven. Ik
observeer mevr. k op haar stemming, gewicht, sufheid of lusteloosheid, dit doe ik omdat het
vaak voorkomt dat de stemming of het gewicht van iemand verandert wanneer hij/zij deze
medicatie gebruikt.
Achtergrondgegevens: Ik weet dat mevr. k wat traumatische gebeurtenissen heeft
meegemaakt in haar leven die waarschijnlijk de angststoornis heeft veroorzaakt.
Observatiedoel en de doelgroep: Ik wil antwoord op de vraag: Is mevr. k haar gedrag
verandert sinds dat mevr. antipsychotica slikt? Waar wil je uiteindelijk een antwoord op
geven? Mevr. K woont thuis en heeft thuiszorg voor het douchen en de medicatie.

Observatievragen: Hoe voelt mevr. k zich? Bijvoorbeeld suf of lichamelijke klachten zoals
misselijkheid. Is mevr. k haar stemming veranderd? Heeft mevr. k nog wel zin om naar het
activiteitenclubje te gaan? Is mevr. k haar gewicht veranderd in de afgelopen tijd?
Manier van observeren: Ik observeer op kwalitatief en continu.
Het observatiemoment: Tijdens het zorgmoment met mevr. k. Op 20-10-2022 om 08:00.
Observatiehulpmiddelen: Ik gebruik mijn zintuigen om mevr. zo goed mogelijk te observeren,
ook zet ik de SNAQ score in en over 1 maand evalueren mevr. k en ik samen over de
gewichtstoename of verlies.
Manier van rapporteren: Ik geef de zo nodige informatie mondeling aan mevr. k. Ook aan
mijn collega’s. Ik rapporteer schriftelijk in het portaal.
