Inleiding
Dit portfolio is geschreven vanuit het kader Sociaal Functioneren 2, profiel zorg met accent GGZ-
agoog binnen de opleiding Social Work aan de Hogeschool van Utrecht. In dit portfolio wordt
aantoonbaar gemaakt wat ik gedaan en geleerd heb ter bevordering van het sociaal functioneren van
cliënten waar ik mee samenwerk op mijn stage. Het sociaal functioneren van een persoon heeft
betrekking op hoe een individu interacteert binnen bepaalde contexten zoals werk, sociale
