Farmacologie1. Middelen die het autonome zenuwstelsel beïnvloeden
1.1. Impulsoverdracht
- Impuls neurotransmitter uit synaps komt in de
spleet terecht door exocytose. Vervolgens gaan
neurotransmitters door diffusie naar het
postsynaptische membraan en bindt daar aan de
aanwezige receptoren. Als reactie hierop worden
de natriumkanalen meer doorlaatbaar.
Concentratie neurotransmitters is afhankelijk van
de impulsfrequentie.
- Twee typen neurotransmitters:
Stimulerend (exciterend). Zorgen voor
afname van het potentiaalverschil (depolarisatie).
Remmend (inhiberend). Zorgen voor toename van het potentiaalverschil (hyperpolarisatie)
afremmende werking op zenuwcel. Kans op actiepotentiaal verkleind.
1.2. . Autonoom zenuwstelsel
- Binnen perifere ZS wordt onderscheidt
gemaakt tussen autonome ZS en het
willekeurige (somatische) ZS
1.2.1. (Ortho)sympatisch zenuwstelsel
- Fight or flight (snellere HR, bronchodilatatie, skeletspieren krijgen meer bloed, glucose vrijgesteld)
- Belangrijkste neurotransmitter: noradrenaline (andrenerge systeem)
- Andrenerge receptoren: alfa- en beta-receptoren
Alfa receptoren -> vaso-Constrictie
Beta receptoren -> vaso- Dilatatie

- Rest and digest (tragere HR en AH, stimulering spijsverteringsstelsel, opbouw glycogeen)

1.3. Indeling van de vegetatieve farmaca
- Vegetatieve farmaca = middelen die inwerken op het autonome ZS
- Sympathicomimetica en parasympathicomimetica (agonisten)
1.3.1. Sympathicomimetica
- Stimulerende werking op sympathisch ZS
- Gewenste effecten: dilatatie pupil, samentrekken bloedvaten, neusslijmvlies ontzwelt door vasoconstrictie,
hogere BD (hypotensie, shock), luchtwegen openen (COPD, astma))
- Bijwerkingen: tachycardie, hartkloppingen, bloeddrukstijging, hoofdpijn
1.3.2. Sympathicolytica
- Remmende werking op sympatisch ZS
- Gewenste effecten: bètablokkers vooral cardiovasculaire aandoeningen, daling oogdruk, relaxatie
spierweefsel blaas, preventie migraine
- Bijwerkingen: dyspnoe, astma-aanvallen, hypotensie, duiziligheid, koude handen en voeten (minder perifere
bloedcirculatie, urineretentie, obstipatie, vermoeidheid
1.3.3. Parasympathicomimetica of chilinergica
- Stimulerende werking op parasympatisch ZS (cholinergica)
- Cholinesterase-inhibitoren
- Gewenste effecten: pupilvernauwing, speekselproductie, progressie dementie afremmen cholinesterase-
inhibitoren
- Bijwerkingen: maagkrampen en diarree, hypotensie
1.3.4. Parasympathicolytica of anticholinergica
- remmende werking parasympatisch ZS ( anticholinergica)
- gewenste effecten: dilatatie pupil, minder speekselproductie, openen luchtwegen, ontspannen darmen,
minder urineverlies (verminderde samentrekking vd blaas)
- bijwerkingen: wazig zien, mydriase, hogere intra-oculaire druk, obstipatie, droge mond, urineretentie,
hartkloppingen, centrale effecten
