Role of the RN

, The role of a dialysis RN is to ensure the patient’s safety throughout the procedure. The

nurse works with the tech to establish the correct protocol with each dialysis patient. When patients

arrive at the clinic, either the tech or the nurse will greet them and lead them to the dialysis area.

Once inside, either the tech or the nurse can weigh the patients and escort them to their chairs. The

tech can set up the machine and get the patient ready for dialysis, but ultimately it is the nurse’s

responsibility to assess each patient. This includes listening to the patient’s lung and heart sounds,

along with taking vitals, such as blood pressure, temperature, and heart rate, both before and after

the procedure. At this time, either the tech or the nurse can insert the needles into the fistula or the

graft, or hook up the machine to a perm cath. It is the nurse’s responsibility to make sure that the

sites are not compromised in any way by listening for a bruit and thrill on a fistula, checking for

placement of a graft and how the blood is pulling from the machine, and then examining the blood

return on a perm cath. Once a patient is placed on dialysis the nurse will continue checking on the

blood pressure that is being taken by the machine every 30 minutes. If the blood pressure is too low

it is the nurse’s responsibility to give the patient fluids through a normal saline bolus form. When the

patient is going through their procedure the nurse will take blood cultures to establish labs on certain

values needed by the doctor. Some of the labs drawn include hematocrit, potassium, calcium, BUN,

and albumin levels. They are also responsible for giving medications on a daily and weekly basis.

Some of the medications administered are Epogen for hematocrit, Hectorol for vitamin D deficiency,

and Veratin, an Iron supplement, due to the fact that most dialysis patients are anemic. If the

patient’s albumin or phosphorus levels are low they will be prescribed a protein bag with phosphorus

additive. The nurse is the only one that can set up the Alaris pump, as doing so is

considered a type of modification. [Des08]
