Introduction lecture
Implementation and evaluation research 5 cases:
- 1) Theoretical D&I models: How can theories support the implementation of
interventions in policy and daily practice?
- 2) Implementation fidelity: How should we implement EBIs in and adapt them to
local contexts?
- 3) At the workplace: What phases can be distinguished in a planned change
trajectory in a hospital?
- 4) Evaluation: Is the intervention effective? Why and how?
- 5) HTA: How can we set up an economic evaluation of the implementation of a
health promoting programme?
Dissemination and Implementation Research in Health (2018); needed in every case.
Lecture 1: Implementation & Evaluation
Planned and systematic approach
Key questions:
- To what extent are Evidence Based Interventions (EBIs) that are developed and
tested according to scientific standards, practically useful and effective?
- What are effective strategies to disseminate and implement EBIs in other
settings?
- And how should policy makers take costs into account when deciding on the
implementation of EBIs?
Effective health promotion effectiveness & dissemination & enabling contexts.
D&I - D&I +
Effectiveness - Placebo Anti-wrinkle cream
Effectiveness + PhD projects and efficacy D&I of effective interventions,
studies according to a planned approach!
- However, D&I of an effective intervention is still dependent on the (enabling)
context. It could be effective in situation A, but not effective in situation B.
- Problematic: only 5-15% of the potential effects of health promotion is reached.
On the shelf: intervention are not really used in practice.
- EBI based on theories and empirical research.
- Tested in efficacy trials
o Efficacy: focus on internal validity.
o Effectivity: effect is tested in the real-life setting.
- Compare with a drug developed in medical research:
o Tested under highly artificial conditions (≠ implementation in real-life).
o Tested with motivated people; strict inclusion criteria (≠ real-life).
