- Blasten = voorlopers /onrijpe volwassen WBC
LEUKEMIE
INLEIDING
- Normale WBC gedaald + veel blasten
- Probleem bij uitrijping stamcel door genetische afwijking
SOORTEN LEUKEMIE
- Acute leukemie: cel kan niet meer uitrijpen
o Moet behandeld worden, vaak dodelijk
- Chronische leukemie: ongelimiteerd delen
o Kan niet behandeld worden, kan er mee leven
- Lymfoïde leukemie: leukemiecellen voortkomen uit voorloper die normaal ontw tot lymfocyt
- Myeloïde: uit stamcel normaal ontw tot granulocyt, rode bloedcel of bloedplaatjes

o Snelle toename leukemische massa
o Kans extreme anemie, leukopenie ( pancytopenie)
- Standaard chemo + SCT
o Inductiefase
Doel: complete remissie induceren + leukemie cellen doden
Remissie = < 5% blasten
Chemo kan moeilijk dr bloed-hersenbarrière -> AML soms nr hersenen ->
chemo via LP vr CZS
o Consolidatiefase
Resultaat behouden
Doel: alle achtergebleven leukemiecellen doden + recidief verkleinen
Evt leukemiecellen in hersenen -> chemo in ruggenmerg / ommaya-reservoir
onder huid van hoofd
o Stamceltransplantatie
- Bijwerkingen conventionele chemotherapie
o Haaruitval
o Misselijkheid en braken
o Diarree
o Vermoeidheid
o Pijnlijke slijmvliezen mond en keel
o Huidreacties
- Supportieve therapie: comfortabeler vr pat, helpt niet bij genezing
Tumorlysis syndroom = snelle tumorafbraak => afbraakstoffen komen vrij (elektrolyten + urinezuur)
=> neerslaan in nier => nierinsufficiëntie
- Rasburicase wnr urinezuur aanwezig, preventie extra hydrateren ( control: urine >7 pH)
- <7 pH: allopurinol -> ↓ vorming urinezuur

o Hoofdpijn en verwardheid (hersenen)
o Benauwdheid (longen)
