PATHOLOGIE:ALTHEROSCLEROSE
DEFINITIES
Arteriosclerose = het normale verouderingsproces van de slagaders waardoor deze minder elastisch en
minder rekbaar worden
Atheromastose = de combinatie van veranderingen in de intima van arteriën, met onder andere focale
opstapeling van verschillende stoffen, en veranderingen in de media. De stapeling
van stoffen leidt tot het ontstaan van een atheroomplaat. We onderscheiden hierin 5
stadia (zie verder).
Aderverkalking = het gaat om een aandoening van de slagaders en NIET van de aders. Bovendien is de
verkalking slecht een zeer bijkomstig gegeven.
Atherosclerose = combinatie van arteriosclerose met daarbij atheromatose.
5 STADIA
1. Stadium I Plaque
‐ Opstapeling van vetten en macrofagen in de gladde spiercellen, die beide op die manier in
schuimcellen veranderen
‐ Deze zijn enkel microscopisch te zien
2. Stadium II Plaque
‐ Schuimcellen stapelen zich op in de intima
‐ De atheromatose is nu ook macroscopisch te zien en noemen we “fatty streaks”
3. Stadium III Plaque
‐ Er verschijnt vrij vet tussen de schuimcellen
4. Stadium IV Plaque
‐ De veteilandjes smelten samen tot een vetlichaampje, een “fatty core”
‐ Deze is omgeven door gladde spiercellen en macrofagen
5. Stadium V Plaque
‐ Door deling van deze macrofagen en gladde spiercellen wordt een echt kapsel gevormd met
collageenvezels en capillairen
‐ Vanaf dit stadium is er vernauwing van het lumen:
o Type V (a) is het klassieke fibro-atheroom
o Type V (b) heeft calcificaties
