PANCYTOPENIE – overzichtALGEMEEN Pancytopenie = vermindering in absolute getallen van de 3 reeksen in het perifeer bloed ➔ anemie + leukopenie + trombocytopenie
Bicytopenie → als 2 van de 3 reeksen gedaald zijn
OORZAKEN VERMINDERDE BLOEDCELAANMAAK IN BEENMERG
• Aplastische of hypoplastisce anemie → beenmergaplasie of beenmerghypoplasie
• Ten gevolge van vitamine B12 - of folaat deficiënte
• Ten gevolge van myeloftyse (leukemie, lymfoom, myeloom, metastatisch carcinoom, myelofibrose, myelodysplastische syndromen)
• Ten gevolge van infectie met inname van beenmerg (bv bij TBC)
VERHOOGDE PERIFERE AFBRAAK
• SLE
• Paroxysmale nachtelijke hemoglobinurie
• Ikv hypersplenisme
o Wordt gekenmerkt door triade: pancytopenie, spenomegalie en verdwijnen van pancytopenie na splenectomie
o Oorzaken:
▪ Lymfomen
▪ Stapelingsziekte bv Gaucher
▪ Felty syndroom (= neutropenie ikv RA)
