INLEIDING • Ouderen (laag-energetisch) & Jongeren (hoog-energetisch)
• Vergrijzing vd populatie -> heupfrcaturen stijgen ongunstige prognose in veel gevallen
DIAGNOSE • KLINISCH: pijn in de lies (+ knie), verkorting en exorotatie been, pijn bij mobilisatie, steunnam
• RADIOLOGISCHE OZ: standaard RX-opnames & CT-scan (uitzonderlijk)
CLASSIFICATIE Gebaseerd op vascularisatie van de femurkop
Intracapsulaire (of femurhalsfracturen) proximale femurfracturen
Garden-classificatie:
type I: onvolledig geïmpacteerde fracturen met valgusstand (‘hoed op de kapstok’);
type II: volledige fractuur zonder dislocatie;
type III: volledige fractuur met dislocatie en varusmisvorming;
type IV: volledige fractuur met varusmisvorming en nagenoeg geen contact meer tss capu
=> I en II stabiele fracturen & III en IV instabiele fracturen
Extracapsulaire (of pertrochantere) proximale femurfracturen
Evans-classificatie:
stabiele fracturen: verloopt volgens de intertrochantere lijn en waar de kleine trochanter
