Het cumulatieve effect van risicofactoren met kleine effectgroottes verhoogt geleidelijk
iemands kwetsbaarheid voor psychische stoornissen tijdens hun leven. Kennis over dit proces
helpt bij het ontwikkelen van verschillende niveaus en stadia van interventies op maat om
risicofactoren te verminderen of beschermende factoren en veerkracht te vergroten. Deze
preventieve interventies kunnen de incidentie, prevalentie en herhaling van psychische
stoornissen verminderen door risicoblootstelling aan te passen en coping-mechanismen te
versterken.
Primaire preventieve interventies zijn bedoeld om risicofactoren aan te pakken en de
geestelijke gezondheid te bevorderen bij personen zonder een klinisch te diagnosticeren
psychische stoornis. Er bestaan drie soorten primaire preventieve interventies:
• Universele preventie. Deze interventies zijn gericht op een hele populatie, ongeacht
het individuele risico (effectiviteit onduidelijk).
o Voordelen: laag stigma (iedereen krijgt dezelfde interventie) en de
mogelijkheid om preventie toe te passen in specifieke settings zoals school en
te integreren in bestaande bredere programma's.
o Nadelen: moeilijkheid om effectiviteit aan te tonen in proeven en over het
algemeen kleine effecten.
• Selectieve preventie. Deze interventies zijn gericht op een subpopulatie waarvan
bekend is dat ze een verhoogd risico lopen op psychische stoornissen (veelbelovend,
maar het is mogelijk dat het gewoon indirect mensen met bestaande
problemen’behandelt').
• Geïndiceerde preventie. Deze interventies zijn gericht op personen die al klinische
manifestaties onder de drempelwaarde vertonen (effectief, maar de opname is te laag
om impact te hebben).
Secundaire preventieve interventies zijn gericht op personen die aan diagnostische
criteria voldoen in de vroege stadia van psychische aandoeningen. Ten slotte zijn tertiaire
preventieve interventies gericht op het voorkomen van terugval bij personen met een
vastgestelde psychische aandoening.
Mensen met psychische stoornissen of handicaps, evenals mensen die al zijn blootgesteld aan
risicofactoren, zijn vaak minder goed in staat zichzelf te verdedigen. Daarom is de kans
groter dat een persoon, zodra die kwetsbaar wordt, verdere risicofactoren verhoogt. Dit leidt
tot een vicieuze cirkel die het faciliterende effect wordt genoemd. Het concept van een
faciliterend effect wordt ondersteund door epigenetische veranderingen die worden gevonden
in de serotoninetransportergenen en de glucocorticoïdreceptorgenen van slachtoffers van
kindertrauma. Deze veranderingen leiden op hun beurt tot neuro-endocriene veranderingen en
veranderingen in de structuur en functie van de hersenen.
Veerkracht is een multidimensionaal construct. Het is het vermogen om zich goed aan te
passen na het ervaren van tegenspoed, trauma of andere stressfactoren. Veel effectieve
interventies bevorderen de weerbaarheid. Het effect van meerdere risicofactoren kan worden
verminderd door veerkracht te bevorderen.
