◼ Laborparameter

- CRP + LDH + PTT + D-Dimere + Hämatokrit + Gesamteiweiß im Urin erhöht
- Leberwerte erhöht (GOT, GPT, Bili)
- Gesamteiweiß im Serum + Quick + Thrombozyten erniedrigt
- Es liegt keine Hypoglykämie vor

◼ Differentialdiagnosen

1) Schwangerschaftsfettleber (~25 SSW)
➔ Seltener aber lebensbedrohlicher Enzymdefekt im Feten → Akkumulation von
Lipiden in den Zellen → Mutter wird mit FS und deren Metaboliten
überschwemmt
➔ Übelkeit, Erbrechen, Müdigkeit, Kopfschmerzen, Oberbauchschmerzen
➔ Im Verlauf oft Ikterus, akutes Nierenversagen (ANV), GI-Blutungen, hepatische
Enzephalopathie
➔ Bili + Transaminasen steigen, Albumin sinkt
➔ Hypoglykämie
➔ HPV neg.
➔ CT, MRT oder Sono (portale Hypertonie erkennbar) zur Diagnostik

2) Virushepatitis
➔ HPV A-E
➔ Pathologische Leberzellnekrose erkennbar (Histo)
➔ Übelkeit, Fieber, Appetitlosigkeit, Arthralgie, Myalgie, Oberbauchschmerzen

3) Thrombotisch-thrombozytopenische Purpura (TTP/HUS)
➔ Thrombozytopenie, hämolytische Anämie
➔ Ausgelöst durch best. Medikamente, Bakterien oder autoimmun
➔ ZNS-Symptomatik (Krämpfe), Verwirrung, Einblutung in die Haut (Purpura,
Petechien), Ikterus, Schwindel, Tachykardie, Ohrensausen, Kopfweh, Fieber

◼ Therapie
- Zeitweise Remission durch Glukokortikoiden (z.B. Dexamethason)
- Als kausale Therapie nur Beendigung der Gravida per C-Sektio möglich, davor
aber Monitoring, beta-Blocker- und alpha-Methyldopa-Gabe bei art. Hypertonie
