, Inleiding
De afgelopen 20 weken heb ik een patiënt gevolgd waarvan het bekend is dat er meerdere
fysieke problemen en aandoeningen aanwezig zijn. De patiënt die ik gevolgd heb is bekend
met rugklachten, waarvoor een DIAM-prothese is geplaatst, Long COVID en astma. Deze
aandoeningen belemmeren haar dagelijks op zowel activiteitenniveau, functieniveau en
participatieniveau. Na een eerste gesprek heb ik geprobeerd de problemen in kaart te
brengen. Hierna heb ik een analyse gemaakt door middel van het positieve
gezondheidsmodel en de HOAC- II. Op basis van de analyse heb ik een adviesplan opgesteld,
samen kijken we iedere twee weken hoe het gaat. Aan het einde van het traject hebben we
een eindevaluatie gedaan. In het verslag hieronder is het proces beschreven.
