We hebben ons bezig gehouden met het schrijven van een ondersteuningsplan voor
een cliënt in de Langdurig Complexe Zorg. Dit ondersteuningsplan is geschreven op
basis van de principes van herstel ondersteunende zorg of de waarden van de
