FYSIOLOGIE: ONTSTAAN EN
ONTWIKKELING VD ZWANGERSCHAP
Het vrouwelijk genitaal stelsel produceert haploïde eicellen (oogenese) ter hoogte van de ovaria, maakt
bevruchting mogelijk ter hoogte van de eileiders en voorziet een goede fysische en hormonale omgeving
voor de implanting en ontwikkeling van het embryo in de uterus.
MENSTRUELE CYCLUS
Processen die leiden tot
- Ovulatie
- Bevruchting
- Implanting van de bevruchte eicel
→ zijn complex en nog steeds niet volledig opgehelderd
Gevolg van een samenspel tussen hypothalamus, hypofyse, ovarium en baarmoederslijmvlies
A. OVARIUM

, Gonadotrofine-releasing hormonen in de hypothalamus
- Stimuleren de hypofyse om FSH (follikelstimulerend hormoon) en LH (luteiniserend hormoon)
vrij te zetten
o Deze hormonen stimuleren op hun beurt het ovarium tot productie van oestrogeen en
progesteron
- Naast deze hormoonproductie heeft het ovarium de functie om eicellen te produceren
(gametogenese).
Lage oestrogeenwaarden (begin en eind van cyclus) → hyopfyse gestimuleerd tot FSH productie →
stimuleert groei van follikels
- Aanmaak oestrogeen neemt toe door follikelgroei → granulosacellen maken inhibine aan
o Gaat FSH productie in adenohypofyse onderdrukken
o → alleen dominante follikel kan verder groeien ⇨ oestrogeenpiek
Dominante follikel heeft de meeste FSH receptoren
Oestrogeenpiek stimuleert de adenohypofyse tot een LH piek en kleinere FSH piek → lokt ovulatie uit
- LH stimuleert lokale productie van prostaglandines en proteolytische enzymen
o → ontwikkeling corpus luteum → produceert progesteron
B. ENDOMETRIUM
Cyclische veranderingen in oestrogenen en progesteron zijn verantwoordelijk voor ingrijpende
veranderingen ter hoogte van het endometrium
Proliferatieve fase
- Oiv oestrogenen → toename vd epitheliale en stromale cellen
o Vergroting vd klierbuizen en endometriale dikte van 1-2 → 10-12 mm
Secretoire fase
- Oiv progesteron → 2tal dagen na ovulatie
- Vermindering vd mitotische activiteit en tortueuze uitgezette klierbuizen met accumulatie van
glycogeen
- Geen bevruchting → dalen concentraties oestradiol en progesteron
o ⇨ menstruatie
