, 3. ANEMIE
Anemie ('bloedarmoede') is een aandoening waarbij de hoeveelheid hemoglobine (Hb) in het bloed
lager ligt dan wat normaal verwacht wordt volgens leeftijd en geslacht van de patiënt.
Daarnaast ook daling van aantal RBC en hematocriet (= volume RBC in verhouding tot totale
bloedvolume).
Algemene symptomen
Anemie leidt tot een daling van O2-transportcapaciteit in het bloed. De verschillende
lichaamsweefsels worden hierdoor dus minder voorzien van O2.
Als er al symptomen zijn, zijn deze vaak atypisch:
− Vermoeidheid, futloosheid
− Bleekheid (is veelal bedrieglijk)
− Hoofdpijn, draaierigheid
Bij ernstige anemie zien we wel compensatiemechanismen in werking treden om de perifere
oxygenatie te bevorderen: tachycardie, tachypnoe,… .
De verschillende soorten anemie kunnen op twee manieren ingedeeld worden, nl. de
pathofysiologische indeling en de morfologische indeling
Pathofysiologische indeling
= indeling van van verschillende vormen van anemie op basis van het pathofysiologisch mechanisme
(oorzaak)
Morfologische indeling
= indeling van verschillende vormen van anemie op basis van het morfologisch aspect (het uitzicht)
van de RBC bij labo-onderzoek.
Het morfologisch aspect van de RBC geeft vaak een sterke aanwijzing voor de onderliggende oorzaak.
− MCV (mean corpuscular volume): = gemiddelde celvolume van een RBC (“grootte”
o Microcytair: RBC te klein
o Normocytair: normal
o Macrocytair: RBC teg root
− MCH (mean corpuscular hemoglobin): = gemiddeld gewicht aan hemoglobine per RBC
(“kleur”)
o Hypochroom: RBC te licht van kleur
o Normochroom: normal
o Hyperchroom: RBC te donker van kleur
