Een sombere stemming kan een normaal verschijnsel zijn, horend bij een rouwproces en bij
verliesverwerking, wanneer het verlies reëel is. Normale somberheid staat in een invoelbare
verhouding tot de aanwezige verlies- en stressfactoren. Deze depressiviteit vermindert wanneer
men erin slaagt het verlies te verwerken en de stressfactoren aan te pakken. Er is pas sprake van
pathologische somberheid of depressie wanneer deze somberheid te sterk is of te lang aanhoudt,
of wanneer de stemmingsontregeling optreedt bij een stimulus die geen depressiviteit
rechtvaardigt.

o 5% van de thuiswonende ouderen (5/100)
o 10% van de RVT-bewoners
o 15-20% van de patiënten met Alzheimer (frequent in het vroeger stadium)
o 15% van de patiënten met een majeure depressie pleegt zelfmoord

In de groene kaders = hoofdcriteria: 1 van beide moet aanwezig zijn om tot een diagnose
van depressie te komen. (depressie stemming of minder interesse)
In TOTAAL 5 criteria aanwezig zijn ( 1 hoofdcriteria, nog 4 nevencriteria) of (2
hoofdcriteria aanwezig, maar 3 nevencriteria nodig).
4 criteria EN depressieve stemming of verminderde plezierbeleving (5 in totaal).

Significante lijdensdruk of beperkingen in het functioneren
Niet het gevolg van middelenmisbruik of een lichamelijke aandoening
Niet te verklaren door schizoaffectieve stoornis, schizofrenie, waanstoornis of andere
psychotische stoornis.
Er heeft zich nooit een manische of hypomane episode voorgedaan.
