Algemeen pijn
1. Niet medicamenteuze adviezen
a. Pijnklachten bewegingsapparaat: fysiotherapie
b. Beperk uitlokkende bewegingen
c. Doorgaan dagelijkse bezigheden, evt aanpassingen maken aan pijnklachten
2. Start paracetamol: 4 dd 2 tabletten 500 mg
3. Voeg oraal NSAID toe of evt. NSAID dermaal
a. Ibuprofen: voorkeur kinderen = 3-4 dd 400-600 mg
b. Diclofenac: bij hoog gastro risico = 2 dd 75 mg
c. Naproxen: bij hoog cardiovasculair risico = 2dd 250-500 mg
4. Voeg tramadol toe: 1-2 dd 50 mg
5. Voeg sterkwerkend opioïd toe:
a. Morfine: 2dd 10-30 mg
Bij spier/gewrichtsklachten:
- Dermaal diclofenac: 3% 2-4 dd
GEEF PPI in geval van:
- Ouder dan 70 jaar
- Ulcus/maagcomplicatie in voorgeschiedenis
- Als twee of meer van de volgende factoren aanwezig zijn:
○ Leeftijd 60-70
○ Ernstig invaliderende RA
○ Hartfalen
○ Diabetes mellitus
○ Hoge dosering NSAID (diclo > 100/dag, ibu >1200/dag nap > 500/dag)
○ Comedicatie zoals antistolling, corticosteroiden, antidepressiva
(PPI remt het enzym van de protonpomp waardoor maagzuurgeproduceerd wordt)
Bij clopidogrelgebruik:
Pantaprazol 20mg tablet
