Primary en secondary survey regelmatig herhalen om problemen tijdig te erkennen.
1. Voorbereiding
Prehospitaal: inlichten ontvangend ziekenhuis, nadruk op behoud van luchtweg, controleren van
externe bloedingen en shock, immobilisatie en zo snel mogelijk transport naar dichtstbijzijnde
geschikte ZH.
Ziekenhuis: klaarzetten en controleren laryngoscoop, tube, infusen, monitor, etc.
Bescherming personeel dmv mondkapje, bril, schort, legging en handschoenen.
2. Triage
A.d.h.v. ABC, bepalen welk ZH geschikt is.
Meerdere slachtoffers: eerst levensbedreigende problemen en multitrauma’s behandelen (capaciteit
van ZH wordt niet overschreden)
Ramp: eerst pt met meeste kans op overleving behandelen (capaciteit wordt overschreden)
3. Primary survey
A Airway: luchtweg behouden met bescherming van CWK
B Breathing: ademhaling en ventilatie
C Circulation: circulatie met controle van bloedingen
D Disability: neurologische status
E Exposure/ Environmental control: pt volledig uitkleden, maar hypothermie voorkomen
direct herkennen en behandelen van levensbedreigende toestanden

Pratende pt A vrij
Inspectie: vreemde lichamen, aangezichts-, mandibula of trachea/larynxfracturen
chin lift, jaw thrust
EMV 8 definitieve luchtweg (tube)
Stabilisatie van CWK totdat fractuur dmv foto is uitgesloten!
Denk aan CWK-letsel bij multitrauma, zeker bij EMV of stomp trauma boven clavicula.
Valkuilen: defecte apparatuur, lekkende cuff, onvermogen tot intubatie na verslapping, mislukken van
chirurgische luchtweg t.g.v. adipositas, obstructie luchtweg bij poging tot intuberen bij larynxfractuur.

Inspectie: AH-excursies, letsels
Auscultatie
Percussie: bloed, lucht
Palpatie: fracturen, subcutaan emfyseem, trachea
Levensbedreigend: spanningspneu, fladderthorax met longcontusie, massieve hemothorax, open
pneumothorax.
Valkuilen: DD A/B-probleem, intubatie bij (spannings)pneu verergert het probleem, controle B na
intubatie (+ X-thorax)
