- Type 1: mastcel gemedieerd, specifieke antistoffen beschikbaar bij specifiek individu
(specifiek IgE tegen specifiek allergeen)
Persoon komt in contact met allergeen -> B- en T-cellen maken antistoffen aan, NOG
GEEN ALLERGISCHE REACTIE bij eerste contact -> volgende keer contact met
allergeen: antistoffen crosslinken via de mestcellen -> degranulatie(vrijzetting
histamine) en allergische reactie
o Dus voor allergische reactie: 3 componenten nodig: IgE specifiek voor het
allergeen, AS gebonden op mastcel, crosslink => activatie van de mastcel
o Na het binden van de AS op mastcel zal er dowstream een complexe cascade
plaats vinden wat er voor gaat zorgen dat de mastcel degranuleert
- Hooikoorts, conjunctivitis, utricaria, angio-oedeem, bronchiaal, GI
- Symptomen:
o Neus: zwelling nasale mucosa, congestie neusloop (allergische rhinitis)
o Ogen: roodheid en jeuk van de conjunctiva (allergische conjunctivitis)
o Luchtwegen: bronchoconstrictie, niezen, hoesten
o Oren: druk op de buis van eustachius
o Huid: jeuk, eczeem of utricaria
o GI: abdominale pijn, diarree, nausea
- Medicatie: steroiden => effecten in activiteit mastcel + histamine receptor blokkers
- Allergeen: voedsel, insecten, schimmels, kattenharen, planten en bomen, AB, latex,…
o Vaccinaties kunnen ook allergische reacties veroorzaken! Daarom na vac de
patient 15 minuten in het oog houden

(Patho)fysologie
- Netelroos: brandnetel (Utrica dioica) -> utricaria/ kwaddels/ galbulten
- Dermografisme: door druk/krabben -> oedeem en roodheid geinduceerd
- Angio-oedeem: ernstige allergische reactie (anafylaxis) (zwelling van hoofd,
gezwollen ogen, lippen,…)
- Hooikoorts
- Allergische neus
- Bijen/wespensteek: anafylaxis
Je kan je laten testen om te achterhalen van wat je allergisch bent. Er wordt een allergeen
oplossing op de huid aangebracht -> indien je allergisch bent zal er zwelling en roodheid
optreden. Je gaat ook een placebo toevoegen (positieve controle) met histamine. Indien je
recent histamine H1 blokkers hebt ingenomen: dan ga je ‘zogezegd’ niet reageren maar dat
is omdat het farmacologische aspect van het geneesmiddel nog actief is.
Kan ook een intradermale injectie van het antigeen inbrengen -> lokaal vasodilatatie,
zwelling en roodheid induceren.
