Evidence-based practice “Primary care home model”
Patient-Centered Medical Home (PCMH) is a model in which providers or groups
provide patients with a single point of access to care. In this type of practice patient and the
coordinators of care have access to providers, continuous care, comprehensive care , family-
centered care, coordinated care, compassionate and culturally effective care. Patients and family
members are able to have a partnership with their personal providers. This concept of patient
centered care is supported by the affordable Care /act (ACA) and the American Association of
Nurse Practitioners (AANP) (Dunphy,Windland-Brown, Porter & Thomas 2015). As an
advanced practice nurse, I hope to be part of a PCMH. Per Medical Economics 2015, this model
is said to produce better patient outcomes, efficient management of patients’, additional provider
incentives and better provider satisfaction(Apostol, 2015). Based on my experience as a Case
Manager, I have noticed that when care is being coordinated by a provider in a PCMH model,
where the provider has in-depth knowledge of the patient, care is associated with lower costs,
higher patient satisfaction, fewer hospitalizations and emergency department visits, and lower
mortality.
Leads to highest quality care and patient outcomes
The patient experience is usually measured by quality care, and not just by provision of
preventive care items on a checklist. In any setting, whether acute or primary care, when
patients are assessed to see if their healthcare entities have the potential to help them to meet
their goals, and the goal of their families, social groups, and the communities. The assessors are
looking at issues that are meaningful to the patients. In patient satisfaction surveys, the patient
experience at a PCMH is said to be fulfilling. One review of the PCMH transition reports that
patients’ recognition has been powerful. Patients report record levels of satisfaction and
