Regardless of your patient's age, you can use any of those subsections. Hearing/Screening Section is a SmartForm (a customizable form in Hyperspace used for gathering clinical and other patient data) that is set up to appear based on the age of the patient. Depending on your setup, this section is in either the Visit Navigator or the Rooming activity SmartSet SmartSets and Order Sets are decision support tools designed to help clinicians work through a visit efficiently and within your organization's recommendations. "SmartSet record" refers to an underlying data structure. A SmartSet record is made available to users as either a SmartSet or an Order Set. SmartSets are typically based on the top diagnoses for the practice. Medical History Significant diagnoses the patient has had in the past Surgical History Significant procedures the patient has had in the past (not necessarily surgeries) Family History Family members' status (alive, deceased, other) and health risks and diseases; an example of when "other" is appropriate is when there is a family member that is no longer in touch, so that person's status is unknown. Family members' status alive, deceased, other Social History A patient's tobacco, alcohol, and drug use, as well as sexual activity Birth History Details about a patient's birth, such as birth length and weight, delivery method, and APGAR scores Obstetric History The (female) patient's pregnancy history Pertinent Negative If a patient or family member has not had a particular diagnosis or procedure, that can be pertinent in helping diagnose a patient or plan the best treatment. It also serves as a hub to go to other activities because many of the SnapShot section headings are links. Define Chart Review Chart Review (also sometimes named Review) is an activity in which users can see a patient's past encounters, lab results, notes, and other parts of a patient's history. They can also apply filters to quickly find information. Define Results Review Results Review displays results in result groups specified by your organization. Use Results Review to quickly access all types of results (as opposed to only labs, for example), choose to "view all" or hand-select what to view, narrow the focus to abnormal or flagged results, and easily view the details of a single result Define Synopsis Synopsis can display a large amount of varying types of clinical data in one view, tracked over time. Define Growth Chart Growth Chart is an activity in which a patient's recorded values for height and weight are used to plot values on various types of growth charts using different reference datasets. When would you use Sidebar Reports When actively engaged in charting on a visit, glance at this report when you: Need a quick overview of the patient's current data, with the possibility of jumping to where you can add to that data. Want to quickly mark allergies, medications, and problem list as reviewed without having to navigate in other activities. When would you use Chart Review? Use it to review "hand-selected" items in the chart. For example, specialists use it before a consult visit to read the primary care provider's progress notes for a referred patient, in order to know about past assessments and treatments. How do you get to Chart Review? In a patient workspace, click the Chart Review activity tab (for some types of encounters, the tab name might be Review). When not in a patient workspace: On the Hyperspace toolbar, click Patient Lookup. From the schedule, select an appointment slot, and click Chart. In Chart Review, under the meds tab a provider can take action on medications: True or False False Why would a clinician utilize Results Review? Use Results Review to: View a variety of results over an extended period of time. Specifically choose what collection of result data to view.
