BOT- EN GEWRICHTSAANDOENINGEN, 1. ALGEMENE INFO
1.1. INLEIDING
Er is een grote groep gewrichtsontstekingen, waarvan reumatoïde artritis de
meest voorkomende en meest invaliderende ziekte is.
1.2. DE BOUW VAN EEN SYNOVIAAL GEWRICHT
Gewricht = scharnierpunten tussen botten
De articulerende* botten worden bij elkaar gehouden door ligamenten en
gewrichtsbanden. De meeste gewrichten bestaan uit een bol uiteinde (de kop) en
een hol uiteinde (de kom).
Kop en kom zijn bekleed met een laag hyalien* kraakbeen (type kraakbeen)
dat stevig in het bot verankerd ligt. Dit kraakbeen werkt als schokdemper,
bovendien is het erg glad waardoor beide gewrichtsvlakken soepel kunnen
bewegen. Het soepel bewegen word nog eens bevorderd door een smerende
vloeistof synovia die zich in de gewrichtsholte bevind (om die reden -> synoviaal
gewricht). Kraakbeen is niet doorbloed, gezien de krachten die soms op het
gewricht uitgeoefend word is dat een voordeel: geen bloedvaatjes beschadigen,
een nadeel is wel dat beschadiging aan kraakbeen langzaam herstelt.
Ligamenten geven het gewricht meer stevigheid, passief sturing aan de
bewegingen in een ‘verkeerde richting’.
Het gewrichtskapsel of capsula atricularis is aan het periost van beide
botten vastgehecht en houdt ze bij elkaar. Het sluit de gewrichtsholte luchtdicht
af waardoor ze tegen elkaar gezogen worden. Het gewrichtskapsel bestaat uit 2
lagen:
Membrana fibrosa (buitenste laag, taai vlies van straf bindweefsel dat
stevigheid verleent, slecht doorbloed -> herstel duurt lang) + membrana
synovialis (binnenste laag, dun vlies dat elastische vezels en bloedvaten bevat,
produceert synovia)
