Naam: Dhr. J.
Leeftijd: 82 jaar
Dhr. woont sinds 2018 in het verpleeghuis De Boomgaerd na een ziekenhuisopname. Dhr. komt uit
een verwaarloosde thuissituatie. Dhr. woonde alleen en heeft geen familie. De ouwe buurman van
dhr. is wel betrokken en is ook de 1e CP van dhr.
Dhr. is vriendelijk en houdt van een grapje. Dhr. is altijd graag op zichzelf geweest en houdt niet van
drukte. Dhr. laat dit ook goed merken, dhr. kan verbaal wat agressief overkomen dan, er is een
benaderplan die goed werkt voor dhr. Dhr. brengt dan ook veel tijd door op zijn kamer. Dhr. kan
soms ook wat somber overkomen en ook negatieve uitlatingen hebben, zie benaderplan. 1 op 1
aandacht vindt dhr. wel fijn. Maak bijvoorbeeld een praatje met dhr. of een wandeling met dhr.
alleen.
Dhr. is ADL afhankelijk. Wordt geholpen met wassen, douchen en aankleden. Dhr. draagt katoenen
onderbroeken, geen incontinentie materiaal. Dhr. heeft een SP katheter en heeft dagelijkse zorg en
observatie nodig. Één keer in de vier weken wordt deze verwisseld door WZB4. Dhr. wilt af en toe de
katheter zak zelf legen.
Dhr. heeft onder eigen tanden en boven een gebitsprothese. Dhr. is brildragend. Dhr. neemt
medicatie geheel in onder toezicht van de verzorgende.
Dhr. eet en drinkt zelfstandig en lust ook alles, geen allergie. Dhr. eet het liefst op zijn eigen
appartement, dhr. wel stimuleren om in de buurtkamer te eten, maar blijft het nee accepteer dit.
Dhr. heeft diabetes type 2, zonder medicatie. Dhr. krijgt daarom light producten aangeboden.
