INSUFICIENCIA RENAL AGUDAEs la disminución rápida de la función renal (ocurre en días o semanas) que ocasiona la
retención de productos nitrogenados y otros desechos que normalmente son eliminados por
los riñones.
Se puede definir de 3 maneras:
Aumento > 0,3 mg/dl de creatinina en < 48 horas.
Aumento > 1,5 veces de la creatinina basal en < 7 días.
Disminución del gasto urinario < 0,5 ml/kg/h por > 6 horas.
Según la duración
Lesión renal aguda < 7 días.
Insuficiencia renal aguda > 7 días y < 30 días.
Insuficiencia renal crónica > 30 días.
Causas
Pre-renal
Se presenta cuando hay una disminución de la perfusión renal ya sea por hipovolemia,
disminución del gasto cardíaco o por una vasoconstricción severa. Entre estas se encuentran
el síndrome cardiorenal, falla cardíaca, shock y fármacos como los IECAs y ARA II.
Renal
Se presenta cuando hay alteraciones en la nefrona como tubulopatías o glomerulopatías.
Entre estas se encuentran las microangiopatías trombóticas, émbolos de colesterol, sepsis y
necrosis tubular.
Post-renal
Se presenta cuando existe una obstrucción del flujo urinario que produce una congestión
retrógrada y una alteración de la función renal. Entre estas se encuentran la ureterolitiasis
bilateral, disfunción vesical y obstrucción uretral.
