This coursework is about both multi agency and multi-disciplinary and how they
come together to work together for both service users and the ethical issues that
they may face when working with both R and Jim.
In health and social care, a multi-agency is a team of health professionals from
different health and social care services. Also, a multi-disciplinary team is a team of
health professionals working together on a patient's case.
One of the agencies is the Clinical Commission Groups which was created in
2012 as part of the health and social care act but then were replaced in the Primary
care trust in 2013. The Clinical Commission Groups is a group of GPs that come
together in every area to deliver excellent quality service to the service users in the
local area. The clinical commission groups also give commission to most planned
hospitals, rehabilitative care, urgent and emergency care, and most community
health service and learning disability services.
The Local Health Boards in Wales was formed in October 2009. The local health
boards in Wales operate seven health boards and three NHS trusts. They are here
to provide the delivery of the NHS in Wales to improve health outcomes and deliver
care effectively with its partners. The Local Health Boards in Wales role is to provide
care near patients houses. Another role is that they can plan and source which will
be able to deliver primary care to hospitals and community health services in the
local area. The members that are part of the Local Health Board are GP’s, Nurses,
Dentists, Pharmacists, Optometrists, and many other health professionals.
Health and Social Care in Northern Ireland was formed in April 2009 it has five
local commissioning groups and five health and social care trusts. It was made to
reform and modernise the management of health and social care services it was also
to integrate provision of services in an efficient, economic, and effective manner. The
role of the Health and Social Care in Northern Ireland is to have effective order in
health and social care services, resources management, performance management
and service improvement. The members of the board are different people in health
and social care services and people from the area, for example finance.
The three agencies that will be a part of the service users' care will be the voluntary
service, the NHS, and the social services. As R has had his stroke the first point of
contact will be the NHS as the consultants will be able to point out, assess him and
find what medication is right for him. As soon as R has met the consultant R would
be referred to a neurophysiologist, who will do an electroencephalography which
does recording of any electrical activity of the brain from the scalp which is used for
those who suffer or suffered from epilepsy which they will refer R back to the
consultant as they have now known what has happened to R the consultant is now
able to give R the right medication for his stroke. The consultant will now refer him to
the pharmacist who will end up giving him the medication. After that R will be sent to
see a physiotherapist as the stroke has affected him which left him unsteady to walk
around the house which has led for him to have a wheelchair for him to move
, around. R would be sent to a speech and language therapist who would help R as
he has difficulty with his speech, but this will let him to adapt and develop his speech
after time. Lastly, R would see a care worker as they will help him with washing and
cleaning also to make his food as he is unsteady to walk around the house also, they
will check to see if R is taking the medication the are prescribed for him. They will
help him to wash his clothes and to do shopping. An agency that will help R is the
stroke association which is a voluntary sector/charity that helps those who have/ had
a stroke to rebuild their lives. This agency can help R to understand that he is not
alone in the condition that he is in instead there is people that are able to help him.
Stroke Associations work with many other organisations for example, Change4Life
which is a government campaign that promotes healthy active lifestyle which
prevents life threatening illnesses for example, Stroke. The stroke association can
help R by fundraising within the local community and nationally which the funds will
be able to help R to work with his everyday life as he is struggling with his condition.
The charity can also help by researching and trying to find diverse ways to improve
their rehabilitations, care, and the treatments.
M5- Assess the benefits of multidisciplinary and multi-agency working for specific
individuals with care and support needs.
A positive of multidisciplinary and multi-agency working for specific individuals with
care and support needs it can give the patient a quick recovery of their conditions
which leads to the patient having less repeated risks. Another positive benefit of
them working together is that it can get variety different of viewpoints from each
professional that is working on the patient’s case. To show this on service. A
negative reason for the multidisciplinary and multi-agency working together is that
the patient may feel overwhelmed as there are many professionals that are on their
case which may cause confusion for the patient as they may not know where to go if
their have an issue. Another negative is that the multidisciplinary and multi-agency
working together is that there may be conflict with the professionals as not all
professionals might know what another professional does for example, for R case he
has a neurophysiologist who may not know someone who works in the stroke
association may do. A positive of multi-disciplinary and multi-agency working
together is that they can look at the patient’s full holistic approach meaning that they
can look at the patient physical, intellectual, emotional, and spiritual needs which
also means that they can give the patient the full work up and give him the right
course of treatment. Another way that the multidisciplinary and multi-agency are able
to benefit the individuals with specific care is to allow the patient to be more involved
in the case and it can help the patients to set goals and aim to what they want to do
and the outcomes that they want to happen. Furthermore, a benefit that can provide
the individuals is to give the patients more access to the service providers that can
help the patients know which service providers can help then with what. Lastly, a
disadvantage of the multidisciplinary team and the multi-agency is that the
professionals may forget to document the time that they have had a meeting, or they
may have not written down the information that they have given the patients.
