o Vaccin PO o.b.v. polysacharide virulentie Ag en vaccin o.b.v. levend afgezwakte bacteriën

Diagnose
o Vroegtijdig met PCR, later in ziekteverloop met serologisch onderzoek
o Rash, artritis/artralgie i.g.v. Chikungunya, korte incubatietijd

o Aantonen van toxine: toxine-bepaling d.m.v. ELISA of PCR op stoelgang of kweek
o Sigmoidoscopie tekens van colitis en pseudomembranen
o Klinisch beeld: secretoir-type diarree, colitis, pseudomembraneuze colitis, fulminante colitis,
paralytische ileus, toxisch megacolon, bloeding, perforatie, sepsis
Behandeling
o Acuut
Stoppen van oorzakelijk AB (indien mogelijk)
Rehydreren
Metronidazol PO (of IV) voor milde gevallen
Vancomycine PO voor ernstige diarree
o Recidieven (frequent 20%; herinfectie of heropflakkering)
Fidaxomycine PO voor recidiverende C. difficile diarree (duur!)
Verlengde kuur met vancomycine PO in afbouwdosis
Faecestransplantatie om de intestinale flora te herstellen
o GEEN loperamide!
Preventie
o Isolatie van symptomatische patiënten
o Grondige reiniging van de kamers
o Strikte handhygiëne
o Restrictief AB beleid
Epidemies zijn een probleem in ziekenhuizen
