COX-1 en -2 verschillende gevoeligheid voor inhibitie (IC50)
1) Anti-inflammatoir: verminderde vasodilatatie, oedeemvorming en

Korte (<6 uur) t1/2: bij acuut probleem, sneller in therapeutisch
venster
Lange (>10 uur) t1/2: bij chronisch probleem
Farmacodynamiek Allen anti-inflammatoir, analgetisch en anti-pyretisch (wel onderlinge
verschillen bv paracetamol NIET anti-inflammatoir)
Analgeticum bij milde tot matige pijn, zonder uitgesproken CZS-
effecten te vertonen
Vooral voor POP, pijn gepaard gaande met inflammatie of
weefselbeschadiging
Indicaties 1) Locomotorische “beschadiging”
2) Pijn (door weefselbeschadiging) (minder voor viscerale pijn)
3) Dysmenorroe
4) Sluiting ductus arteriosus
5) Koorts
Bijwerkingen Agv COX-1 inhibitie:
1) GI:
- Probleem: >65 jaar, combi NSAID met GC of alcohol, langdurig,
voorgeschiedenis peptische aandoeningen à mylena = alarm!
à met maaltijd, PPI, misoprostol + NSAID is cytoprotectief (maar
wel diarree en nausea), maagsapresistent tablet (nog wel effect
via systemische weg)
2) Bloedplaatjes:
- Inhibitie TXA2 synthese à verlengt bloedingstijd
3) Renaal en CV:
- P met chronisch verminderde nierfunctie à AKI
- P met chronisch hartfalen, levercirrose of KI à oedeem,
overvulling en acuut orgaanfalen
