⟹ het hart voert zelf bloed weg aan de organen
en de kleine circulatie, maar hoe krijgt het hart
zelf bloed? Dit is via de kransslagaders, de left
anterior descending artery, de left coronary
artery, de circumflex artery en de right
coronary artery. Als de rechter kransslagader
zou dichtslibben dan is er nog altijd een kans tot
overleven maar als de linker slagader dicht zou
slibben dan zeer weinig, omdat deze ook de
circumflex en de LAD voedt.
2. hartlongmachine
⟹ het doel is het overnemen van de pompwerking van het hart en het overnemen
van de gasuitwisselingsfunctie van de longen
- Hierbij is er mogelijkheid tot het opwarmen en afkoelen van het bloed van de
patiënt
- En de mogelijkheid tot opzuigen en tijdelijk stockeren van bloed
- De onderdelen:
o Pomp (roller- of centrifugaal): neemt de pompfunctie
over van de linker hartkamer
o Oxygenator (membraan): zorgt voor de gastuitwisseling (O2 en CO2)
o Warmtewisselaar: laat koelen en opwarmen van de patiënt toe
▪ Als het koel is, is er minder zuurstof gebruik, soms moet heel het
lichaam koelen als we de circulatie willen stilleggen
o Canules en tubings: maken de verbinding tussen toestel en patiënt
▪ Canules is voor hartlongmachine die in v. cava inferior gaat
o Reservoirs en filters
▪ Gesloten: geen bloed-lucht interface (of compartimenten gaan
inflameren) en het vermijdt doorpompen van lucht
▪ Open: heeft hogere capaciteit, kan filter incorporeren en kan
gemakkelijker volume meten

, 2.1 werking van de hart-long machine
1. Priming: het gehele hart-long machine incl. de tubing moet gevoeld worden
met vloeistof (bloed, plasmavervangende of fysiologische zoutoplossing)
a. Hierdoor kan machine aangekoppeld worden zonder ‘aderlating’
b. Hemodilutie (Hct = 20) maakt het bloed minder stroperig bij lage
temperatuur
2. Heparine + protamine: toediening van heparine (3mg/kg IV) maakt het bloed
volledig onstolbaar met ACT >400
a. Priming verdunt het bloed
b. Continu de stolling checken (ACT) is de activated clonting time
c. Op het einde wordt dan een antidote heparine protamine gegeven
2.2 hart-longmachine myocardprotectie
⟹ is bescherming van het hart tegen schade door de ischemie
- Topical cooling: koude fysiologische zoutoplossing (4°C) of ijssludge rondom
het hart
- Cardioplegie: inspuiten van koude cardioplegische (hartverlammende)
vloeistof in de kransslagaders voor hart stil te leggen
- Hypothermie en fibrillatie (fibrilleren van hart verbruikt minder zuurstof)
- Intermittente klem + fibrillatie en reperfusie
- Kristalloïde cardioplegie
- Bloedcardioplegie
- Elektro-mechanische stilstand:
o Koude temperatuur, voor elke 10°C lager is er 50% minder
metabolisme
o Afwezigheid van distentie: afwezigheid, geen energie
3. coronaire bypass operatie
3.1 coronair sclerose
- Etiologie: atheromathose (slagaderverkalking) van de kransslagaders
- Fysiopathologie: vernauwingen in de kransslagaders geven ischemie
(zuurstofnood) van de hartspier
- Klinische symptomen: angina pectoris en hartinfarct (hartspier sterft af)
- Diagnose: ECG, fietsproef, scintigrafie, coronarografie, CT-coronaro
- Interventioneel:
o PCI: percutane coronaire interventie (+/- stent)
o CABG: coronary artery bypass graft
▪ Instent respinose: wanneer er vernauwing is in de stent
vanbinnen
▪ Beschikbare venen:
• V. saphena magna
• V. saphena parva
• A. mammaria (achter de ribben)
• A. radialis
o Vaak ook de allen-test
