⟹ is de meest voorkomende complicatie van een premature geboorte, de
zuurstofbehoefte is langer dan 28d na de geboorte en is veelvoorkomend bij extreme
prematuren (<32w)
- Te weinig ontwikkelde longen
- Te weinig goed ontwikkelde longblaasjes
- Minder goed doorbloede longblaasjes
- Bronchospasmen
- Ernstige pulmonale hypertensie
- Surfactant kan helpen
- Longen zijn gevoeliger voor infectie
- Beschadigde longen, littekenweefsel door hulp bij de AH (langdurige
beademing/cpap op NICU)
o Langdurige zuurstofnood
2. pleurapneumonie
⟹ is een pneumonie met ophoping van vocht tussen de pleurabladen zoals
transsudaat, exsudaat, pus, lymfe, bloed, meestal emyeem (pus in de pleuraholte,
meestal met pneumokokken, stafylokokken of haemophilus)
- 3 stadia:
o Exudatief met nog weinig cellen
o Fibrinopurulent met massaal granulocyten en fibrineschotten met
‘loketvorming’
- Symptomen:
o Hoge zuurstofnood en toegenomen kortademigheid
o Hoog CRP
o Pijn op de borst
o Buikpijn
o Lange pneumonie
o Aanhoudende koorts ondanks AB
o Gedempte percussie en ademgeruis
o Soms verminderde thoraxbewegingen
o Prikkelbaarheid
- Diagnose:
o Thoraxfoto: beschaduwing en
concave bovenbegrenzing
o Echoscoop: dikte vochtschil,
aard vocht, fibrineschotten en
vaststellen punctieplaats
