Humaan Herpes Virus type 8 of KSHV
GEMEENSCHAPPELIJKE EIGENSCHAPPEN
primaire infectie blijven latent aanwezig [in zenuwweefsel (HSV, VZV), of in lymfoid
weefsel (EBV, HHV6, 7 en 8)] reactivatie spontaan of door stresstoestand (koorts, zon)
Cellen komen los van elkaar en er komt een klein epidermaal blaasje.
BESMETTING: GEÏNFECTEERDE SECRETIES
HSV type 1:
o Primaire infectie: : contact met besmet speeksel
Asymptomatisch (90-95%)
Gingivostomatitis (vooral bij kindjes: 1-5 jaar)
Pharyngitis (jonge volwassenen)
Na primo-infectie trekt het virus zich terug in ganglion van nervus trigeminus
o Reactivatie: herpes labiales (meestal)
HSV type 2
o Primaire infectie: besmette genitale vloeistof
Kan vanaf eerste sexueel contact
Vaker asymptomatisch, maar als ze symptomen geven zijn deze zwaarder
dan bij een reactivatie
Balanopostitis, vulvovaginitis, proctitis
Er kunnen ook blaasjes optreden rond uterus, perineum,..
o Reactivatie: minder ernstig en korter verloop
HERPETISCHE GINGIVOSTOMATITIS: PRIMAIRE
INFECTIE

, Vesikels die makkelijke openbarsen en dan aanleiding geven tot oppervlakkige ulceraties =
erg pijnlijk
Regionale klierzwelling, algemene symptomen
Restitutio binnen de 2 weken
Soms hospitalisatie nodig met IV vocht toediening
HERPES LABIALIS: REACTIVATIE
Virus trekt zich terug in sensibele ganglia, maar kan terug
gereactiveerd worden in een stresstoestand
Reactivatie virus (UV, infecties, premenstrueel,…)
Prodromale symptomen: pijn, jeuk, tintelingen
Kleine gegroepeerde vesikels op een erythemateuze basis
Behandeling zo snel mogelijk opzetten, omdat de behandeling vooral efficiënt is wanneer het
virus nog aan het repliceren is (dus in prodromale fase)
Restitutio na ongeveer 7 dagen
HERPES SIMPLEX TYPE 1: REACTIVATIE
Ook ovv herpes thv de wang (DD cellulitis)
o Cellulitis vaak met koorts, warm en pijnlijk
o Herpes reactivatie is ook pijnlijk, maar voelt niet warm aan en gaat
ook niet echt gepaard met koorts
o Wisser nemen, blaasje openprikken en materiaal op kweekbodem
kan DD maken!
Hier een patiënte met een reactivatie thv de wang vesikels hebben
aanleiding geven tot erosies
HERPES GENITALIS: HERPETISCH BALANOPROSITITIS
Primaire vorm
o Incubatie: 2-20 dagen
o Meestal ook asymptomatisch!
o Prodromale fase: pijn, jeuk, dysurie
o Regionale klierzwelling, algemene symptomen (koorts, malaise)
o Erythemateuze macula vesikel pustel ulceratie
Blaasjes, vaak thv penis(schacht), maar kunnen ook
rond perineum, proctitis,…
o Restitutio na 2 tot 3 weken
Reactivatie
o Gelijkaardige kliniek, maar minder ernstig en korter verloop
o De letsels zijn soms zeer beperkt zodat de patiënt het niet
merkt
o Deze situatie is vooral bij de vrouw een bron van nieuwe
infecties
o Maar de patiënt kan ook buiten periodes van zichtbare afwijkingen de infectie
doorgeven („viral shedding‟)

GENITALE HERPES: ONDERSCHATTE (ONBEKENDE) PROBLEMEN
Psychische impact, besmettelijkheid voor de partner
Talrijke niet herkende opstoten
Asymptomatische vrijzetting van HSV2: “viral shedding”
Zwangerschap (seronegatieve moeder, aterme, met seropositieve partner)
o Primo-infectie tijdens bevalling en vaginale bevalling baby kan besmet worden
tijdens passage door geboortekanaal neontale besmetting!! (mogelijks fataal)
HERPES SIMPLEX: ABNORMALE VERLOOPSVORMEN EN COMPLICATIES
Herpetische keratoconjunctivitis • eczema herpeticum • herpes gladiatorum (treedt vaak op
bij gevechtssporten door intens huid-huid contact) • herpetische witvinger
Levensbedreigende complicaties: herpetische encefalitis • herpes neonatorum (weinig risico
wanneer vrouw een recidief heeft) • herpes in immuungedeprimeerden
Erythema exsudativum multiforme (EEM) = huidreactie op medicatie of herpes
schietschijfletsels
HERPETISCHE KERATOCONJUNCTIVITIS
Directe infectie van het oogslijmvlies door contact met besmet secreet
Meestal HSV1, tenzij neonatale herpes
Unilaterale of bilaterale keratoconjunctivitis met ooglidoedeem (primo-
infectie)
Allemaal blaasjes, die evolueren naar korstjes rond de oogleden en ogen
Tranen, fotofobie en preauriculaire lymfadenopathie
Complicaties: ulceratie cornea (keratitis) met verlittekening, eventueel blindheid
HERPETISCHE WITVINGER
Door rechtstreeks contact met besmet secreet (vb. speeksel)
Vooral aangetroffen bij medisch en paramedisch personeel, primaire
gingivostomatitis en duimzuigen (auto-inoculatie): maar dit
veronderstelt een aangetaste huidbarrière bij de persoon!
Heel pijnlijke aandoening de huid is gezwollen, rood en heel pijnlijk: gegroepeerde
visikeltjes zijn te zien
DD: cellulitis (uitbereiding van een bacteriële infectie), maar de blaasjes
zijn anders en de pijn is veel meer uitgesproken bij witvinger
ECZEMA HERPETICUM
Verspreide infectie (niet noodzakelijk primair) bij voorafbestaande
huidaandoeningen (meestal atopisch eczeem)
