Dermatofyten (gekeratinizeerd weefsel): huid, haren, nagels
Gisten: huid & slijmvliezen
DERMATOFYTEN
1. Trichophyton spp. (huid, haar en nagel)
o Vb. T. Rubrum (humane schimmel waarbij de infectie gebeurt van mens naar
mens): treffen we het meeste aan bij infecties van de huid en de nagels
Chronische afwijkingen, die klinisch minder duidelijk zijn dan de
inflammatoire afwijkingen bij animale schimmel
2. Microsporum spp. (huid en haar)
o Vb. M. Canis (animale schimmel infectie van dier naar mens)
Afwijkingen zijn veel duidelijker dan de humane schimmels
3. Epidermophyton spp. (huid en nagel)
o Vb. E. floccosum
GISTEN
1. Candida spp. = commensaal! geven dus geen problemen bij gezonde personen!
o Vb. C. albicans
2. Malassezia (Pityrosporum) species:
o Behoren tot de normale commensale flora van de huid, maar geven enkele
problemen bij overgroei!
o Pityriasis Versicolor, cfr les papulosquameuze aandoeningen
DERMATOFYTEN
Indeling volgens lokalisatie
o Niet behaarde huid
Tinea corporis
Tinea cruris
Tinea faciei
o Behaarde huid
Tinea barbae of Tinea capitis
o Sterk gekeratiniseerde huid
Tinea pedis of Tinea manuum
o Nagels Onychomycose
TINEA CORPORIS
Dermatofyt van de niet telogeen behaarde huid, exclusief handen en voeten, maar ook
exclusief de lies en het gelaat
Meestal typische afwijkingen: erythematosquameuze haarden met perifere randactiviteit
(schilfering, papels, pustels, vesikels) en centrale genezingstendens;
o Afwijkingen zijn meer uitgesproken in de rand van het letsel, dan centraal
o Je moet je materiaal voor rechtstreekse microscopie of kweek dus uit rand halen!
Bij chronische afwijkingen of verkeerde therapie met lokale corticoïden minder typische
kliniek (DD eczeem, psoriasis).

, Sterk inflammatoire (animale schimmel) tot meer chronische afwijkingen (humane
schimmel).
Perifere schilferende rand, met centrale genezingstendens! Anulaire letsels smelten samen tot polycyclische letsels.
Dermatofyten zijn afhankelijk van keratine, blijven dus beperkt tot het epiderm!
Bij humane schimmels zijn het meer chronische afwijkingen met minder duidelijke boord
TINEA CRURIS
= Infectie van de mediale zijde van de dijen en de liesplooien.
o Zal bijna nooit uitbreiden naar het scrotum, maar wel via perineum naar dorsaal
Vaak begeleidend tinea pedis en onychomycose
Meestal jonge mannen (vooral bij watersport)
Vaak T. rubrum die aanleiding geeft tot chronische infecties

Afwijkingen rechts veel duidelijker dan
links (= verschil tussen animale en
menselijke schimmel)
Bij de chronische vorm zijn de afwijkingen
veel minder spectaculair!
TINEA CAPITIS (BEHAARDE HUID)
Vaak binnen gezinnen door gemeenschappelijke gebruik
van oa. washandjes
Microsporie:
o Geeft geen volledige kaalheid, wel “stoppelveld” haartjes breken af, maar boven
huidoppervlak
o M. Canis (familiale epidemies), vroeger ook M. Audouinii (pandemieën)
Jonge zwerfkatjes = belangrijke infectiebron voor tinea capitis door M. canis
o Positieve fluorescentie: groene fluorescentie bij wood-lamp
o Vooral in kleine familiale epidemie
Trichophytie:
o Wel volledige kaalheid: haartjes breken af thv huid
o Klinische diagnose is veel minder duidelijk dan bij microsporie
o T. Tonsurans (geeft geen inflammatie), T . Rubrum
o Negatieve fluorescentie: geen groene fluorescentie!

o Inflammatie+++ (karbonkelachtige afwijkingen)
o T. Mentagrophytes, T. Verrucosum
o Bron: vee → veeartsen, landbouwers
