Esclerosis MúltipleENFERMEDAD DESMIELINIZANTE DEL SNC (NO AFECTA AL SNP)
ETIOLOGÍA
Autoinmune (contra Oligodendrocitos).
Forman mielina en SNC.
→Asociada a Hla Dr2 y DQ.
EPIDEMIOLOGÍA
2da causa de disfunción nerviosa en jóvenes (la primera causa de un trauma).
● Episodio previo de Neuritis Óptica.
● Familiares de primer grado.
CLÍNICA.
Clínica en brotes y Remisiones.
Puede presentar cualquier sintomatología del SNC (motora,sensitiva,
visual,cognitiva y cerebelosa).
● Neuritis Óptica en el 60% , con escotoma central, dolor ocular.
● Vértigo.
● Oftalmoplejia intranuclear.
→Hipoestesias y parestesias (Síntomas más frecuentes).
Fenomeno de Uhthoff:La clínica se exacerba cuando hace mucho calor
(fiebre,infecciones,playa).
Signo de Lhermitte: El paciente siente una descarga eléctrica a nivel de la
columna cervical por una extensión.
DIAGNÓSTICO.
1.Resonancia Magnética contrastada con gadolinio: encontramos lesiones
desmielinizantes hiperintensas en T2.
-Criterios de Mcdonald.
● En LCR encontramos bandas oligoclonales de IgG (Depositos de
destrucción de mielina).
TRATAMIENTO.
Brotes Agudos: Pulsos de esteroides (metilprednisolona) o plasmaféresis.
Modificadores de la enfermedad:
1era línea
-Interferon B
-Acetato de Glatiramer (En pacientes con pérdida de tejido cortical y menor
inflamación).
2da línea
-Azatioprina
-Ciclofosfamida
