A. Service Idea

In Texas from 2002-2007, the sixth leading cause of death was illnesses related to

diabetes ("Texas Diabetes Fact Sheet," 2011). There are three types of diabetes: type 1, type 2,

and gestational. Over 90% of the diabetics are type 2 (Public Health Impact: Diabetes, 2019). A

diabetes education program provides people with the information, skills, and confidence to

appropriately manage their diabetes and lead a healthy happy lifestyle. The educational program

can also delay a prediabetic for years of developing the disease. By providing a diabetes

education program, I believe it will greatly improve the health of our community. Over the past

ten years the percentage of diabetics in Texas has increased. In 2017, approximately 2,323,220

Texans had diabetes which was 11.4% of our population (Diabetes Prevention Program Sites

Compared with Diabetes Prevalence and Ration of Primary Care Physicians in Texas, 2019).

Texas is one of the ten states that together account for over half of the national cost of diabetes,

which was at an estimated $327billion in 2017 (Public Health Impact: Diabetes, 2019). It is

estimated that every year in Texas there are 182,000 people newly diagnosed with diabetes (The

Burden of Diabetes in Texas, 2014). This number indicates a clear need for a program such as

this.

Type 2 diabetes is a progressive disease that is preventable if there are lifestyle changes

including but not limited to smoking cessation, losing weight, increasing physical activity,

maintaining a normal blood pressure, and having a healthy diet (Public Health Impact: Diabetes,

2019). The National Diabetes Prevention Program coordinates public and private organizations

to help delay the progression from prediabetic to type 2 diabetic. These programs have been

shown to reduce the prevalence of diabetes by 58% for those that are at an increased risk of the

disease and/or delay the onset by as much as four years (Public Health Impact: Diabetes, 2019).

Classes will be once a month at our local hospital to provide support for the patients and

the community it serves. Our classes would offer diabetes information, teaching using different

methods, return demonstration, coupons for supplies from manufacturers, information on self-

care, referral lists for other specialties related to diabetic illnesses, nutrition information, meal

prepping, as well as being a support group. The amount of possibilities for the educational

program are endless.

The classes would be open to the public but would target prediabetics and diabetics along

with their caregivers or spouses/family. We would provide general diabetes information in the

form of teaching, slideshows, return demonstrations, pamphlets, or handouts. All the

information we provide would be available in English and Spanish. We teach patients about the

different types of insulin or oral medications and give them information on their specific type,

including signs to watch for with blood sugar that is too high or too low. Return demonstration

to assess their knowledge and confidence of correctly performing blood sugars and administering

medications. Our program would provide nutritional information on foods appropriate for their

diet, as well as teaching how to prepare the food or providing different recipe options. Age

appropriate exercise options would also be taught to improve physical activity.

The CDC and AMA have been working since 2014 to prevent diabetes on a national

level. PreventDiabetesSTAT is a national initiative that aims to increase collaboration between

physicians, care teams, and patients. The aim is to stop the progression from prediabetic to type

2 diabetes (Smith, 2017).
