1. Volair plaatletsel of hyperextensie trauma PIP gewricht
Palmaire zijde is boven
1. Proper collateraal ligament
2. Accesoir collateraal ligament
3 en 4. Palmaire ligamenten
Accesory ligament zit alleen vast aan volaire plaat dus komt bij
extensie op rek
Proper ligament zit vast van boven naar onder op het volgende
kootje, dus komt bij flexie op rek
A. Anamnese
Door scheuren van de palmaire ligamenten (die houden oorspronkelijk hyperextensie tegen), ontstaat
instabiliteit van het PIP gewricht. Daarnaast kan ook een contractuur ontstaan bij een te lange immobilisatie
naar flexie.
- Het niet meer actief kunnen strekken van de PIP
- Wordt vooral gezien bij volleyballers en handballers
- Zwelling thv PIP gewricht
- Lokale pijn thv PIP gewricht
B. Onderzoek
- Extensieprovocatietest: de patiënt beweegt de vinger naar extensie (AROM). Daarna brengt de therapeut
het PIP gewricht naar hyperextensie
> Positief: pijnprovocatie en grotere bewegingsmogelijkheid tov gezonde zijde
2. Collateraalband letsel PIP en MCP
A. Anamnese
Collateraalband letsel kan ontstaan tgv een trauma:
- Gevoel van instabiliteit
- Zwelling
- Pijn bij bewegen
Daarnaast kan een contractuur ontstaan in de gewrichten bij verkeerd spalken. Zo kan een flexiecontractuur
van het PIP gewricht ontstaan en een extensiecontractuur van het MCP ontstaan. Deze contracturen hebben te
maken met de collaterale banden die niet in de rekpositie zijn geïmmobiliseerd en dus verkorten.
B. Onderzoek flexiecontractuur PIP en DIP
- Extensieprovocatietest (accesory ligament wordt oa getest omdat het ook voor zijwaartse stabiliteit zorgt)
- Varus/valgus stabiliteittesten in 0˚ extensie en 30˚ flexie: De therapeut brengt een varus/valgusstress op
het te testen interphalangeale gewricht.
> Positief: kleinere bewegingsmogelijkheid tov gezonde zijde
Bij 0˚-30˚ wordt het accesory ligament getest, bij 30˚ -eind het proper ligament
