TOSTOS = neurovasculair compressiesyndroom. Een aantal structuren van en rondom de plexus (zenuwen, venen
en arteriën) raken bekneld, wat kan komen door:
- Hypertrofie m. scaleni
- Gezwollen ligamenten (lig. costoclaviculare)
- Gezwollen weefsel ten gevolge van schade of repetitieve bewegingen (voornamelijk boven het hoofd)
- Trauma
- Tumor
- Excessieve callus vorming bij een gebroken clavicula of eerste rib
- Bij een breuk een uitstekend gedeelte
- Halsrib
Vaak onderste delen plexus aangedaan, dus klachten bevinden zich vaak in dermatoom/myotoom C8/TH1
> Costoclaviculaire ruimte: ruimte tussen eerste rib en clavicular, waar de neurovasculaire bundel loopt
> Voorste scalenuspoort: ruimte tussen het claviculaire deel van de m. sternocleidomastoideus en de m
scaleni anterior. Hierdoorheen lopen v. subclavia, de kleine arteriën en de grote lymfevaten.
> De achterste scalenuspoort: ruimte tussen de m. scalenus anterior en de m. scalenus medius. Door deze
poort loopt de a. subclavius en plexus
> Coraco-pectorale poort: ruimte tussen verbinding van de m. pectoralis minor met proc. coracoideus en
bovenste ribben. Hier loop ook de neuro vasculaire bundel doorheen.
A. Anamnese
- Doof gevoel
- Pijn/ tintelingen (vaak rond dermatoom C8/ Th1)
- Spiertjes in hand verminderd, dus handcoördinatie verminderd en vermoeid gevoel (= moeite met
fijnmotorische handelingen)
- Bewegingen boven het hoofd zijn provocerend
Aanvullende vragen voor n. ulnaris: ’s Nachts klachten, tijdens maximale flexie arm?
Aanvullende vragen voor cervicaal radiculair syndroom: nekbewegingen provocerend?
Aanvullende vragen voor carpaal tunnel syndroom: tintelingen en pijn dermatoom n. medianus ((duim,
wijsvinger, middelvinger, half ringvinger), druk op de basis van de hand is provocerend, duimmuis
atrofie (vooral in later stadium)?
B. Onderzoek
Doel 1: uitsluiten dat er sprake is van een letsel aan de n. ulnaris (cubital tunnel syndrome)
- Maximale flexie elleboog + vingers hyperextensie, 1 minuut vasthouden
> Positief: herkenbare klachten
- Op n. ulnaris slaan/ palperen in cubitale tunnel
> Positief: herkenbare klachten
Doel 2: uitsluiten dat er sprake is van een carpaal tunnel syndroom
- Phalen wrist extension test: De therapeut brengt de hand in maximale palmaire flexie en houdt de pols
hier gedurende 60 seconden.
> Positief: ontstaan of toename van de paresthesieën in het gebied van de n. medianus
Sens: 0,88/ spec: 0,93
- Tinel-Hoffmann sign N. Medianus in carpaaltunnel: De therapeut brengt de te onderzoeken hand naar
dorsaalflexie met de te onderarm in supinatie. Vervolgens tapt de onderzoeker met vinger (of reflex
hamer) op de n. medianus t.h.v. distale polsplooi.
> Positief: ontstaan of toename van de paresthesieën in het gebied van de n. medianus
Sens: 0,66/ spec: 0,83
- Median nerve compression test: “ UGH als hierboven, maar nu drukt de therapeut tot maximaal een
minuut lang op de n. medianus
> Positief: ontstaan of toename van de paresthesieën in het gebied van de n. medianus
