AC-gewricht- Horizontale adductie (linkerarm in rechter broekzak) is pijnlijk en/of onmogelijk;
- Zwelling
- Stijfheid
- Liggen op aangedane zijde is pijnlijk
- Provocatie bij eindstandige anteflexie en rotaties
- Beweeglijkheid schoudergordel ernstig beïnvloed
- Trauma en daardoor acute pijn; denk aan luxatie (door de val krijg je compressie in het gewricht waardoor
horizontale adductie pijnlijk is;
- Pijn boven op schouder/ lokaal AC gewricht;
- Pijn kan soms uitstralen naar de arm of nek;
- Komt veel voor bij sporters die worpsporten doen en andere activiteiten boven het hoofd
- Leeftijd: tieners – 50 jaar
>50 jaar, denk aan osteoartritis, waarbij pijn bij bewegen en bewegingsbeperking aanwezig is/ artrose
waarbij vooral de eindstandige anteflexie is beperkt
B. Onderzoek: inspectie (let op hoogstand clavicula)
Doel 1: aantonen dat er sprake is van AC nocisensoriek
- AC ‘pinpoint pain’ palpation test / Paxinos’ sign: palpeer de gewrichtsspleet van het AC gewricht.
> Positief: circumscripte pijn rondom AC gewricht, afweerreactie, vaak is er zwelling aanwezig
- AC resisted extension test: therapeut brengt de arm van de patiënt in 90° horizontale adductie en vraagt
vervolgens om de arm tegen weerstand naar buiten te duwen (horizontale abductie)
> Positief: circumscripte pijn rondom AC gewricht
- ROM
> Positief bij bewegingsbeperking (vooral eindstandige anteflexie), pijn en crepitaties (duidt op artrose)
Doel 2: aantonen/ uitsluiten dat er sprake is van een laks AC kapsel
> Moet wel uit anamnese een trauma blijken.
- Piano-key sign: bij inspectie is meteen al een hoogstand van de clavicula te zien, de therapeut duwt het
distale einde van de clavicula naar caudaal en laat het weer opveren
> Positief: de clavicula veert op
Tossy 1: lig acromioclavicularie overrekt of gedeeltelijk geruptureerd
Tossy 2: lig acromioclaviculaire geruptureerd, lig coracoclaviculare overrekt of gedeeltelijk geruptureerd
Tossy 3: typisch pianotoets fenomeen – lig. acromioclaviculaire – en coracoclaviculaire
NB. Bij langer bestaande AC luxaties (> 1jr.) wordt er een stevige fibreuze verbinding gevormd tussen het
acriomion en de clavicula. Hoewel er een hoogstand is waar te nemen is het pianotoetsfenomeen minder
uitgesproken aanwezig tot zelfs afwezig. Functioneel vormt dit nauwelijks belemmeringen voor de patiënt.
> Verwachting Tossy 2:
Doel 3: aantonen dat er sprake is van coracoclaviculaire nocisensoriek
A. Anamnese:
- Pijn rond het coracoid bij bewegingen richting anteflexie;
- Veel voorkomend bij frozen shoulder.
Mobiliteit GH verminderd -scapula beweeglijkheid wordt teveel - clavicula verplaatst minder naar craniaal -
ruimte tussen proc. coracoideus en clavicula wordt minder
- Glenohumerale mobiliteit ROM
> Positief: bewegingsbeperking, vooral bij anteflexie (treedt pijn op)
- Bewegingscoördinatie scapula
• Herhaalde anteflexie en herhaalde abductie
> Positief: afwijking scapulohumerale ritme (bijv. winging)
- Stenvers 4: clavicula
