Casus sepsisBij aankomst kind met koorts bij ALL
- PAC aanprikken
- Urinekweken, bloedkweken en mond/keel/anus kweken.
- Start breedspectrum AB, wanneer kweken bekend specifiek
- CRP, leuko’s, trombo’s, DIFF (differentiatie, kan uit het bloedbeeld bepaald
worden. Bij een DIP granulocyten <0,5.).
- Kind met koorts, lage leuko’s, een DIP, laag CRP; maak je je iets minder
zorgen dan bij een torenhoog CRP, omdat het dan waarschijnlijk viraal is. Bij
een bacteriele infectie zie je een hele hoge CRP, is gevaarlijker.
- UP: 1 ml/kg/uur tot 30 kg.
- Vitale functies controleren: aansluiten monitor, want mocht ze een sepsis
ontwikkelen dan kan je veranderingen opmerken.
- Mond doet pijn: systemische pijnbestrijding, pcm en zo nodig morfine. Geen
lidocainegel, omdat je dan geen gevoel meer hebt en nog meer stuk kan bijten
o Goede pijnschalen gebruiken
Packed cells:
- 10-15 ml/kg in 3 tot 4 uur.
- Controles: vitale functies
- Transfusiereactie zie je tijdens het inlopen
Ze krijgt sepsis doordat:
- Door cytokines van bacterien gaan vaten wagenwijd open staan, niet zo zeer
omdat je te weinig volume hebt, maar wel doordat je een te lage
vaatweerstand hebt.
- Je gaat zuurstof geven om zo veel mogelijk zuurstof te laten circuleren. Ze
heeft in verhouding te weinig circulerend volume, dus je wilt aan dat volume zo
veel mogelijk zuurstof toevoegen. In acute situatie 15L via NRM
Vaatvulling:
- NaCl 0,9%
- 20 ml/kg in een paar minuten tot maximaal 30 minuten (max 500ml per keer).
Bij koorts: eerst anitbiotica. Maar wanneer koorts niet reageert op antibiotica, is de
kans groot dat ze een schimmel hebben. Bij invasieve beademing heb je meer kans
op schimmels in de longen.
Mondslijmvliezen
- Schoonhouden
- Regelmatig bevochtigen
- Goede pijnmedicatie
- Goede observatie op veranderingen (extra schimmelinfectie?)
- Goede voedingstoestand wanneer het kind gestabiliseerd is
- Mond knapt meestal op, wanneer bloedwaarden weer gaan stijgen.
