HC anesthesiePatient komt op preoperatieve poli:
- Gezondheidstoestand in kaart brengen en eventueel optimaliseren
o Vaak wordt diuretica gestopt, omdat het bloeddruk verlagend is en je juist
vocht wil geven tijdens de OK.
- Voorlichting
- Anesthesietechniek
o Epidureel, loco-regioneel (specifieke zenuw verdoven), spinaal.
- Er wordt een risico inschatting gemaakt. ASA1 tot en met ASA5.
o A1: gezonbde patient
o A5: patient zal zonder ingreep waarschijnlijk overlijden binnen 24 uur
Anesthesieteam:
- Anesthesist:
o Medisch specialist
o Eindverantwoordelijk
- Anesthesiemedewerker
o Anesthesiemedewerker = anesthesieverpleegkundige = anesthesie-assistent
o Kan verpleegkundige zijn
o Onderhoudt zelfstandig de anesthesie
o Belt anesthesist bij problemen
Algehele anesthesie:
- 3 pijlers:
o Bewustzijnsverlaging
o Pijnstilling
o Spierrelexantia
- Leiden tot areflexie: alle reflexen worden uitgeschakeld
Bewustzijnsverlagende medicatie
- Propofol, thiopental, etomidate
- (S) ketamine
- Midazolam
- Gewenst effect: patient valt in slaap
- Ongewenste effecten: ademwegobstructie (door verslapping spieren),
ademdepressie, hypotensie (vaten gaan open staan, hart werkt minder goed), pijn bij
injectie
Pijnstilling
- Sufentanil (400x zo sterk als morfine), fentanyl, alfentanil, remifentalil
- Morfine piritranmide vaak op de afdeling
- Gewenst effect: patient is pijnvrij
- Ongewenst effect: ademhalingsdepressie, hypotensie, misselijkheid en braken
Spierrelaxantia:
- Rocuronium, cisatracturium, pancuronium
- Succinylcholine
- Gewenste effect: totale spierrelaxatie
- Ongewenst effect: tachycardie, hypotensie, pijn bij injectie
