Infecties
Acute urineweginfecties (cystitis)
ANI kent een zelflimiterend verloop
Kinderen jonger dan 12 jaar: verhoogd risico op gecompliceerd verloop
en schade aan de nieren.
Verhoogd risico op gecompliceerd verloop
o Tijdens zwangerschap, bij mannen, diabetes,
immunogecompromitteerde patiënten, anatomische afwijkingen van
nier, neurologisch blaasstoornissen en verblijfskatheters.
Belang van urinestaal: belangrijk voor cultuur en antibiogram alvorens
te starten met AB.
Eerste keuze middel: nitrofurantoïne oraal (Furadantine). Andere AB
hangt af van antibiogram.
Acute pyelonefritis
Blind starten met antibiotica na afname van urinestaal voor cultuur en
antibiogram.
Zwangere vrouw: advies van gynaecoloog
Bij zorgvrager met onderliggend urologisch lijden: advies uroloog.
Onderscheid tussen pyelonefritis en prostatitis is moeilijk!
Hospitalisatie
o Bij sepsis I.V. behandeling.
o Bij kinderen jonger dan 12 jaar.
Eerste keuze middel: Levofloxacine of ciprofloxacine P.O.
Acute prostatitis
Blind starten met antibiotica na afname van urinestaal voor cultuur en
antibiogram.
Moeilijk onderscheid tussen acute pyelonefritis en prostatitis
o Behandeling verloopt identitiek, maar behandelduur bij prostatitis
kan langer zijn.
Opletten voor Soa’s die betrokken kunnen zijn: chlamydia, gonokokken en
mycoplasma.
o Indien SOA aanwezig: SOA behandelen en partner ook (contact
tracing).
Eerste keuze middel: Levofloxacine of ciprofloxacine P.O.
o Bij chlamydia: azithromycine.
Orchido-epididymitis
Bij kind: meestal virale oorsprong, dus geen AB nodig. Wel opletten voor
torsio testis.
o Vanaf 12 jaar en volwassen: wel AB.
