, Inleiding


Voor beroepsprestatie 3.1 ‘Verdiepen in de Branche’ heb ik 3 zorgwijzers van de instelling
bijgehouden. Dit houdt in dat ik elke dag de rapportage heb geschreven en het zorgdossier heb
bijgehouden. Daarvoor heb ik ook elke dag de controles bij moeder en kind gedaan, die hierin
vermeld worden. Ook heb ik voor deze BP de andere formulieren ingevuld, zoals het formulier voor
het JGZ en het LIP-formulier. Ik wist niet zeker of dat moest, maar heb het voor de zekerheid gedaan.
Voor elk gezin heb ik zorgproblemen uitgewerkt via de PES-methode. Twee problemen bij de moeder
en twee problemen bij het kind. De PES-methode houdt in: Probleem, Oorzaak, Symptomen. Daarbij
heb ik nog toegevoegd; interventies, planning volgens de SMART en het resultaat.

Van elk gezin heb ik gegevens verzameld en een casus geschreven, een samenvatting van de eerder
geboden zorg geschreven, de wetgeving, en afspraken, en procedures en protocollen die van
toepassing zijn, precies volgens de checklist van BP 3.1.

De ingevulde zorgwijzers plus alle documenten heb ik op papier als bijlage. Deze hoefde ik niet
allemaal in te scannen omdat anders het bestand te groot wordt.
