- Je kent de micro-organismen die betrokken zijn bij het ontstaan van parodontitis en kan het proces van
parodontitis uitleggen aan de hand van de eigenschappen van betrokken micro-organismen.
- Je bent bekend met de specifieke, niet-specifieke en ecologische plaque hypothese en kan de verschillen
tussen deze hypothesen benoemen.

Parodontale ziekte: is een aandoening van de ondersteunende structuren van de tanden inclusief de gingiva,
parodontaal ligament en alveolair bot

De twee belangrijkste categorieën zijn
1. Gingivitis (ontsteking aan het steunweefsel)
2. Parodontitis (ontsteking aan het parodontium, met afbraak)

PARODONTIUM
Het parodontium bestaat uit:
1. De gingiva
2. Het wortelcement
3. Het parodontaal ligament
4. Het alveolaire bot

DE SULCUS

De ecologie van de gingivale sulcus:
De ecologie van de gingivale sulcus:
verschilt van plaats tot plaats in de mond
o In de sulcus zal je meer anaerobe bacteriën vinden dan in andere plekken
in de mond
o De sulcus bevat gingivale creviculaire vloeistof (GCF: afkomstig vanuit het
serum/ bloedbaan) met onder andere eiwitten en componenten van het
immuunsysteem uit het bloed

, Kolonisatoren in de gingivale sulcus

Bacteriën hechten aan verschillende oppervlakten in de sulcus
bijvoorbeeld:
o Pocketepitheel
o Het glazuur
o Bij aanhechtingsverlies à worteloppervlak
o En tussen het pocketepitheel en de tand. Dit noemen we
flowing plaque ‘’drijvende’’ bacteriën in de vloeistof

Het aantal bacteriën in een gezonde sulcus varieert en is
relatief laag 103 - 106

In het creviculaire epitheel: zien we onder andere de
peptostreptococcus micros

En bij het blootliggende tandcement zien we vooral de:
o Pionier species zoals de streptococcen en de actinomyces spp
o Secundaire kolonisatoren: prevotella en porphyromona spp

KENMERKEN VAN BOVENGENOEMDE BACTERIËN

Streptococcen
Streptococcen kan je indelen en dat wordt op basis gedaan van hun hemolytisch
vermogen. Hemolytisch vermogen = hemolyse (afbreken van bloedcellen). Hemolyse is
het afbreken van rode bloedcellen in een voedingsboden met bloedagar door daarop
groeiende micro-organismen. Dus wanneer je een plaatje hebt met bloed agar kan
hemolyse dus de bloedcellen afbreken. Hier heb je verschillende gradaties in:

o α- hemolyse: is het gedeeltelijke hemolyse (produceren waterstofperoxide en
daardoor oxideren de bloedcellen)(groene agar)
o β-hemolyse: de complete hemolyse (heldere agar)
o γ-hemolyse: Geen hemolyse (rode agar)

orale streptococcen**
