De afgelopen vier maanden heb ik stage gelopen bij Vierstroom Zorg Thuis op locatie X. Voor
deze opdracht heb ik een cliënt gekozen om een zorgplan voor te maken. Aan de hand van
informatie over mevrouw (mw.) heb ik twee diagnoses opgesteld en daarbij passende
doelen bedacht. Om deze diagnoses te kunnen verbeteren en doelen te kunnen behalen heb
ik interventies gezocht om toe te passen. Ik heb het proces bijgehouden en beoordeeld aan
de hand van een evaluatie of er verbetering te zien is of niet.
