Introductie in de medisch microbiologische diagnostiek

Casus Daan, 18 maanden oud;
- Hoge koorts, bewustzijnsdaling, luierpijn (ongewoon huilen bij verwisselen van luier)
- Nekstijf, zitten kleine (niet-wegdrukbare), paarsrode vlekjes op de benen (petechiën en
purpura)
- Verdenking: meningitis, door meningokok
Neisseria meningitidis – bacterie, in het nederlands meningokok.
 Er worden dan neutrofiele granulocyten verwacht in het liqour.
 Gramnegatieve coccen, die in dublo liggen.

Gramkleuring
Grampositief – donkerpaars gekleurd
Gramnegatief – roze gekleurd

Het verschil ontstaat door een verschil in opbouw van
de celwand, waarbij een dikke laag
peptidoglycaan zorgt voor een grampositieve
kleur.
Daarnaast geeft het informatie over de vorm en
soms ook de ligging van de bacteriën

Niet alle bacteriën worden zichtbaar in een
gramkleuring:
- Mycoplasma – hebben geen celwand
en wordt dus niet zichtbaar.
- Spirocheten – (spiraalvormige bacteriën), zijn te dun om zichtbaar te worden met
gramkleuring. Donkerveldmicroscopie of specifieke immuunfluorescentie is nodig om deze
bacteriën zichtbaar te maken. Voorbeelden:
o Borrelia burgdorferi – Lyme
o Treponema pallidum – syfilis
o Leptrospira spp. – de ziekte van Weil
- Mycobacteriën – hebben wel een celwand, maar deze is ondoordringbaar voor de
gramkleuring door aanwezigheid van wasachtige vetzuren. De Ziehl-Neelsen-kleuring
(kleuring voor zuurvaste staven) kan deze bacteriën wel zichtbaar maken. Voorbeelden:
o Mycobacterium tuberculosis – tuberculose
o Mycobacterium leprae – lepra

Bij een kleuring van mycobacteriën zie je soms een ‘ghost bacteria’ 


Casus Vincent, 25 jaar oud;
- Homoseksueel, meerdere seksuele contacten gehad (condoom geknapt)
- Hulpvraag: HIV

HIV – Human Immunodeficiency virus. Dit virus veroorzaakt ernstige immuunsuppressie waardoor
uitindelijk andere infecties optreden en AIDS (Acquired Immune Deficiency Syndrome) ontstaat.

- 3 weken na het incident wordt hij ziek; koorts, malaise, spierpijn en vergrote lymfeklieren in
oksels en liezen.
