Mijn naam is . Ik ben werkzaam bij de Thuiszorg Patyna in Workum. Sinds Februari 2020 volg
ik de opleiding MBO-verpleegkundige. Hiervoor heb ik verschillende opleidingen gevolgd in
de zorg, waaronder Verzorgende IG en Maatschappelijke zorg. Dit examen gaat over
verbindend samen werken met andere betrokkenen en zorgverleners. In dit examen ga ik
laten zien hoe ik heb samen gewerkt met verschillende disciplines en betrokkenen,
waaronder: de cliënt, Diëtiste, fysiotherapeut , familie en mijn collega’s.
