Op de casus van dhr. P. pas ik de stappen toe van een planmatige aanpak van
gezondheidsbevordering: gezondheidskundige analyse, doelen opstellen, interventies
bepalen en uitvoeren, monitoren interventies en een evaluatie.
Ik ga eerst een gezondheidskundige analyse maken over het risico op verstikking. Daarna ga
ik aan de hand van de analyse doelen en interventies opstellen, deze ga ik uitvoeren met
een bij passend meetinstrument geschikt voor de cliënt. Dit ga ik vervolgens monitoren en
evalueren.
